SLIS will hold the second of this academic year’s three-part SLIS Lecture Series, featuring the latest research of the faculty. It will be held on 09 December 2015, Wednesday, 12nn-4pm, at the SLIS Rooms 3 and 4. There will be five lectures discussing different areas of LIS and ARM, namely:

What to do?

• You must be able to do all the following and submit the required documents by 08 January 2015, Thursday.

• If original documents (i.e., TOR, NSO Birth/Marriage Certificate) have been submitted, and if you have already taken the OCG Aptitude Exam, proceed to the SLIS College Secretary’s Office for your Referral Slip to the UP Health Service. 
• Proceed to the UP Health Service for the medical exam (present the Referral Slip).
• Submit the photocopy of your Medical Certificate to the SLIS College Secretary’s Office and fill out the Student Directory so you will be issued the College Admission Slip.
• Submit your College Admission Slip and the original copy of your Medical Certificate to the University Registrar so you will be issued the University Admission Slip. 

Submit the University Admission Slip to the SLIS College Secretary’s Office.

List of Accepted BLIS Applicants for 2nd Semester AY 2014-2015

WHAT TO DO?

• Confirm your slot on or before 12 December 2014, Friday. 
• You must be able to do all the following and submit the required documents on or before 8 January 2015, Thursday. 
• For those on MRR, AWOL, or LOA status, you must be able to clear all your deficiencies by 09 January 2015, Thursday.

• Get Notice of Admission from the SLIS College Secretary’s Office as soon as possible. 
• Secure Clearance and Permit to Transfer from our previous college and submit these to the SLIS College Secretary’s Office.
• Fill out the Student Directory (2 copies) at the SLIS Office.  Submit College Admission Slip, Student Directory, TCG, Clearance and Permit to Transfer to the Office of the University Registrar. 

You will be issued the University Admission Slip.  Submit this to the SLIS College Secretary’s Office.
